A partire dai colloqui iniziali con una studentessa affetta da disturbi psicosomatici e depressivi, viene elaborata una comprensione psicoanalitica degli incontri iniziali. La scena iniziale del primo colloquio condensa già la psicopatologia centrale: un aggrapparsi all’oggetto primario, mai sperimentato in modo sicuro come presente dal paziente. L’autore delinea lo sviluppo di alcune teorie psicoanalitiche riguardanti il colloquio iniziale e dimostra la loro importanza specifica come conoscenza di base per la situazione clinica nei seguenti ambiti: la “posizione diagnostica”, la “posizione terapeutica”, la “scena d’apertura”, il “controtransfert” e “l’introspezione liberamente fluttuante” dell’analista. Ricerche più recenti fanno riferimento a “qualità di processo” della relazione analitica, come la “sincronizzazione” e l’“autoefficacia”. Quest’ultima cerca di descrivere dopo quanto tempo tra le sedute di intervista i processi interiori costruttivi o distruttivi prendono piede nel paziente e che significato può avere per la decisione sul trattamento che segue. L’insieme di questi fattori può portare a stabilire un’indicazione differenziale orientata al processo che tenga conto anche del fatto che il confronto con la paura dei processi inconsci di scambio è specifico della professione psicoanalitica.
