Viene rivisitato criticamente il concetto di controtransfert a partire dalla definizione di Freud sino alle posizioni più recenti. Mentre Freud concepiva il controtransfert come un ostacolo al lavoro analitico, in sèguito si è fatta strada una concezione allargata o "totalistica" che include non solo il "transfert dell’analista" (dovuto a un suo eventuale "residuo nevrotico"), ma tutte le reazioni, anche realistiche, nei confronti del paziente; alcuni autori, quali Paula Heimann e Heinrich Racker, hanno teorizzato che questo tipo di controtransfert non è di ostacolo ma è utile perché permette di conoscere "direttamente" l’inconscio del paziente. Queste posizioni vengono criticate, e viene discusso criticamente anche il concetto di "amore analitico". Tramite una revisione della letteratura sulla ricerca empirica sul controtransfert, spesso ignorata dagli psicoanalisti, viene mostrata la sua utilità per definire i concetti e verificare le ipotesi, e argomentato che è più giustificata una concezione non estrema ma moderata di controtransfert.;
