A new treatment approach for PTSD: High-Intensive Narrative Exposure Therapy (HI-NET)

Current international guidelines recommend Trauma-Focused Therapies (TFT) to treat Post-Traumatic Stress Disorder (PTSD). However, non-response and dropout rates have shown to be substantial. TFT, delivered in a highly intensive dose may lead to a more efficacious symptom reduction and lower dropout rates. This article describes an inpatient treatment program with Narrative Exposure Therapy (NET), called High-Intensive NET (HI-NET). The program consists of sixteen NET sessions during two weeks, combined with art therapy and yoga. After describing the treatment program, we present two case reports of patients who received HI-NET. We measured amongst others symptoms of PTSD and depression, sleep quality and general psychopathology at pre-treatment, post-treatment and at four months’ follow-up. The two patients showed that applying HI-NET is a promising approach to append to the existing way of conducting NET. One patient reported a slight increase in symptoms at post-treatment, but was fully recovered of PTSD at follow-up. The other patient recovered gradually, with full recovery at follow-up. The other measures also showed important improvements at the end of treatment and at follow-up. Concluding: NET in a high intensive form is applicable and feasible. Our two cases showed an important improvement in PTSD symptoms and in general psychopathology. More research is needed with larger patient groups to determine efficacy and efficiency more properly.

Le attuali line guida internazionali consigliano le Terapie Trauma-Focali per il trattamento del Disturbo Post-Traumatico da Stress (DPTS). Tuttavia, la resistenza alla cura e l’incidenza di drop-out sono tuttora significativi. Le terapie trauma focali proposte in un format intensivo potrebbero favorire la riduzione dein sintomi e dimi-nuire l’incidenza di drop out più efficacemente. L’articolo descrive un programma di tratta-mento in regime di ricovero che utilizza la NET in un format intensivo chiamato High-Intensive NET (HI-NET). Il protocollo è costituito da sedici sedute NET nell’arco di due settimane che si integrano con l’arte terapia e lo yoga. Una volta descritto il trattamento, so-no illustrate due casi clinici di pazienti che sono stati coinvolti nel protocollo. Sono stati valutati i sintomi di DPTS, depressione, qualità del sonno e il quadro psicopatologico generale sia prima sia dopo l’intervento e a 4 mesi dalla fine del trattamento. HI-NET è un approccio promettente; un paziente ha mostrato un lieve peggioramento dei sintomi a chiusura del tratta-mento, ma una totale remissione al follow-up. L’altro paziente è migliorato gradualmente con una totale guarigione al follow-up. La valutazione ha mostrato un generale miglioramento alla chiusura e al follow-up. In conclusione, la NET in formato intensivo è possibile e sostenibile. I due casi hanno mostrato miglioramenti significativi sia rispetto ai sintomi post-traumatici sia al quadro psicopatologico generale. Sono necessari altri studi con un campione più ampio di pa-zienti per determinare in modo più robusto efficacia ed efficienza.
